The Child and Youth Services (CYS) Central Registration Office for Fort Story is located in building 303 Cape Henry Road. It is a one-stop registration point for the Child Development Center (CDC), Family Child Care (FCC), School Age Services (SAS), Middle School/Teen (MS/T), Sports & Fitness (S&F), SKIES, and Youth Services (YS) programs. We work on a walk-in or appointment basis, with each registration taking approximately 60 minutes.

REGISTRATION
Registration is required for all CYS programs. There is an Army mandated non-refundable annual registration fee per child or per family. Priority is given to all single military and DA civilians and dual working military and civilian families assigned to Forts Eustis and Story regardless of branch of service. A registration packet must be completed prior to registering with any CYS program. This registration packet may be obtained from the central registration office during operational hours. During registration, the following documents must be verified before your child may start in a CYS program:

. Proof of immunizations
. Registration Form
. USDA Eligibility Form
. Child's birth certificate, military ID, or passport
. Child's social security number
. Current LES or pay statement
. DD Form 2652, Application for DOD Child Care Fees
. Three local emergency designees and their telephone numbers
. Family Care Plan for single and dual military couples
. Current physical (done within the last 1 year)
. Individual Education Plan for children who have special needs
. Special Needs Declaration. If your child has a special need (i.e. asthma, learning disability, allergy, ADHD, etc) a screening by the Special Needs Director and Resource Team will be required before starting.

In addition, parents must attend an activity orientation to the program the child will be attending.

There is a waiting list for most child care programs. It is strongly urged to place your child's name on the waiting list as soon as possible. Parents who are PCS'ing to Forts Eustis or Story or expectant mothers are encouraged to place their child's name on the "future needs" waiting list. DD Form 2606, Request for Care Record, DOD Child Development Program, may be used to submit your child's name for placement on the waiting list.

IN AND OUT PROCESSING
Provides information for military and their families who are new to the
base and provides assistance to those families with school age children
when moving to a new location.

RESOURCE AND REFERRAL
Provides assistance to patrons who need information on off-post child care options and child care availability at other military installations.

PARENT LIAISON SERVICES
Coordinates parent educational opportunities via workshops, parent education materials and links to websites concerning parents and children.

COMMUNITY LIAISON SERVICES
Coordinates volunteer services and training for adults and teens who wish to become certified babysitters.

....BABYSITTER TRAINING AND REFERRAL
Offers babysitting training courses and maintains a referral list for parents interested in this child care option.

....VOLUNTEER SERVICES
Provides a centralized point of contact for volunteers within the CYS
Delivery system. Recruitment, management, training, and referrals are
done through the OS office.

OUTREACH CARE AND SUPERVISION OPTIONS

....SHORT TERM ALTERNATIVE CHILD CARE (STACC)
Offers on-site child care supervision for special group functions in
approved sites.

....VOLUNTARY CHILD CARE IN A UNIT SETTING (VCCUS)
Offers training for unit volunteers to provide child care in a group or home setting, enabling units or other groups to hold meetings and/or functions.
